Parkinson’s disease is a progressive neurological condition that primarily affects motor function. While there is currently no cure, the strategies for managing Parkinson’s disease have evolved significantly. Modern treatment focuses on maintaining quality of life, reducing tremors, and improving mobility through a combination of medication, physical therapy, and advanced surgical interventions.

Early diagnosis remains a vital component of successful management. When symptoms such as resting tremors, muscle stiffness, and slowed movement (bradykinesia) are identified early, a consultant neurologist can tailor a plan to slow the impact on daily activities.

Pharmacological Treatments and Medication Management

The primary goal of medication is to replenish dopamine levels in the brain. Levodopa remains the most effective treatment. It is often combined with carbidopa to improve absorption and reduce side effects.

Other pharmacological options include:

Dopamine Agonists: These mimic the effects of dopamine in the brain.

MAO-B Inhibitors: These help prevent the breakdown of existing brain dopamine.

COMT Inhibitors: These extend the effect of levodopa therapy.

Managing these medications requires precise timing. As the condition progresses, some patients experience periods where medication wears off before the next dose is due. Adjusting the dosage or introducing extended-release formulas can help maintain a steady state of symptom control.

Advanced Therapies and Surgical Options

For patients who no longer respond effectively to oral medications or who experience severe involuntary movements, advanced therapies offer a different path.

Deep Brain Stimulation (DBS) is a well-established surgical procedure where electrodes are implanted into specific parts of the brain. These electrodes produce electrical impulses that regulate abnormal impulses. This can significantly reduce the need for medication and stabilise motor fluctuations.

Other modern approaches include continuous subcutaneous apomorphine infusions or levodopa-carbidopa intestinal gel. These methods provide a more constant delivery of medication than traditional tablets.

The Role of Physical and Occupational Therapy

Managing Parkinson’s disease is not limited to clinical prescriptions. Physical activity is a powerful tool in preserving motor function. Specialist neuro-physiotherapy focuses on gait, balance, and strengthening exercises to reduce the risk of falls.

Occupational therapy assists patients in adapting their home and work environments. This might include using assistive devices or learning new techniques for tasks that have become difficult due to tremors or stiffness. Speech and language therapy is also beneficial if the condition begins to affect voice volume or swallowing.

Managing Parkinson’s Disease with Dr Paviour

Nutrition and sleep hygiene play a supportive role in overall wellbeing. A diet high in fibre and adequate hydration can help manage common symptoms like constipation. Regular aerobic exercise has also been shown to have potential neuroprotective benefits.

Support from a multidisciplinary team ensures that the treatment plan remains responsive to the changing needs of the patient. This team typically includes neurologists, specialist nurses, and therapists. Regular reviews allow for the fine-tuning of treatments to ensure independence for as long as possible.

If you are concerned about symptoms or wish to discuss management strategies, Dr Paviour provides expert neurological assessment and personalised care plans.

Most people associate Parkinson’s disease with visible movement issues like tremors, stiffness, or slow movement. While these motor symptoms are central to the condition, they often appear later in the disease progression. Research now shows that many patients experience a variety of non-motor symptoms years before a tremor ever develops. Recognising these early signs is essential for securing a timely diagnosis and starting a proactive management plan.

The silent early signs of Parkinson’s DISEASE

The earliest symptoms of Parkinson’s disease can be subtle. They do not involve movement, so patients and even some general practitioners may attribute them to ageing or other minor health issues. However, when these symptoms occur together, they provide important clues about neurological health.

Common early indicators include:

Loss of sense of smell: Many patients notice a reduced ability to smell long before motor issues begin.

Sleep disturbances: Vivid dreams, talking, or physically acting out dreams can indicate early changes in the brain.

Constipation: Persistent digestive issues are a frequent early complaint, as Parkinson’s can affect the nerves controlling the digestive tract.

Mood changes: Feelings of anxiety or a low mood can emerge without a clear external cause.

Recognising non-motor symptoms

As the condition progresses, non-motor symptoms often become more prominent. These issues can impact your daily life just as much as movement difficulties, yet they are frequently under-reported during consultations.

Dr Dominic Paviour focuses on identifying these hidden symptoms to provide a more complete picture of a patient’s health. These may include:

Cognitive changes: Some individuals experience difficulty with multitasking, planning, or word-finding.

Blood pressure fluctuations: A sudden drop in blood pressure when standing up can cause dizziness or fainting.

Fatigue and pain: Persistent tiredness that does not improve with rest, or unexplained aches in the joints and muscles, are common.

Bladder issues: An increased urgency to use the bathroom, particularly during the night, can be a symptom of the condition.

The importance of a specialist assessment

Parkinson’s disease is a complex condition that affects everyone differently. There is no single test for the disease, so a diagnosis relies heavily on the clinical expertise of a neurologist. Dr Dominic Paviour, a Consultant Neurologist in London, specialises in the diagnosis and long-term management of Parkinson’s and other movement disorders.

During a consultation, Dr Paviour performs a thorough evaluation of both motor and non-motor symptoms. He uses this detailed assessment to distinguish Parkinson’s from other neurological conditions that may mimic its symptoms. This careful approach ensures that you receive an accurate diagnosis and a treatment strategy tailored to your specific needs.

Managing Parkinson’s DISEASE with expert guidance

A modern approach to Parkinson’s care focuses on improving quality of life. While medication is often used to manage motor symptoms, Dr Paviour also addresses the non-motor aspects of the condition through a combination of clinical therapies and lifestyle adjustments.

Early intervention allows for better symptom control and helps patients maintain their independence for longer. By working with a specialist who understands the full spectrum of the disease, you can navigate your diagnosis with confidence and support.

Book a consultation with Dr Dominic Paviour

Parkinson’s disease is a progressive neurological condition that affects movement. Parkinson’s disease symptoms develop gradually over time, and they can vary significantly from person to person. While many people associate the condition with a tremor, it can also manifest in other ways, affecting a person’s balance, coordination, and daily activities. A timely diagnosis and a comprehensive treatment plan can help manage these symptoms and maintain a good quality of life.

Key Parkinson’s disease symptoms

The primary symptoms of Parkinson’s disease are motor-related. They include:

Tremor: An involuntary shaking, which often starts in one limb, typically in a hand or finger. This tremor is most noticeable when the hand is at rest.

Bradykinesia: A slowness of movement, which can make simple tasks like walking or getting dressed feel difficult and time-consuming.

Rigidity: Stiffness in the limbs or trunk. The muscles become stiff, limiting a person’s range of motion and sometimes causing pain.

Postural instability: This can lead to problems with balance and coordination, increasing the risk of falls.

In addition to these motor symptoms, Parkinson’s disease also has non-motor symptoms, such as a loss of smell, sleep problems, depression, and cognitive changes.

Treatment options

While there is currently no cure for Parkinson’s disease, a range of treatments can help manage symptoms and improve a person’s quality of life. A neurologist will tailor a treatment plan based on a person’s symptoms, age, and general health.

Medication: Doctors can prescribe medicines to help with the primary motor symptoms. These drugs work by increasing or replacing dopamine, a brain chemical that diminishes in people with Parkinson’s.

Therapies: Physical therapy can help with mobility, balance, and flexibility. Occupational therapy can help a person with daily tasks, and speech therapy can help with speaking and swallowing difficulties.

Surgical treatments: For some people with advanced Parkinson’s, a doctor may recommend surgical options. Deep Brain Stimulation (DBS) is a procedure that involves implanting electrodes in the brain to send electrical impulses that can help control some motor symptoms.

Expert care with Dr Dominic Paviour

Receiving a diagnosis and starting a treatment plan requires the expertise of a specialist. Dr Dominic Paviour is a consultant neurologist with a special interest in movement disorders, including Parkinson’s disease. He is also the clinical lead for Movement Disorders at the Atkinson Morley Regional Neurosciences Centre.

The ability to move freely and purposefully is something many of us take for granted. For those who experience a movement disorder, this can become a daily challenge. Movement disorders are a group of neurological conditions that cause problems with movement, including involuntary movements, reduced or slow movement, or an inability to move. While Parkinson’s disease is the most well-known, this field of medicine encompasses a wide range of conditions that each affect a person in a unique way.

What are movement disorders?

Movement disorders affect the speed, fluency, quality, and ease of a person’s movement. They are typically divided into two categories:

Hypokinetic disorders: These are characterised by a slowness or lack of movement. The most common example is Parkinson’s disease.

Hyperkinetic disorders: These cause excessive, involuntary movements. These can include tremors, dystonia (involuntary muscle contractions), chorea (involuntary, jerky movements), and tics.

A neurologist is a medically qualified doctor who specialises in disorders of the nervous system, including movement disorders.

Focus on Parkinson’s disease

Parkinson’s disease is a progressive neurological condition that affects a person’s movement. Symptoms develop gradually and can vary from person to person. The most common symptoms include:

Bradykinesia: A slowness of movement that can make simple tasks difficult and time-consuming.

Rigidity: Stiffness in the limbs or trunk, which can be painful.

Postural instability: Problems with balance and coordination, which can increase the risk of falls.

While there is currently no cure, treatments can help manage the symptoms and improve a person’s quality of life.

Beyond Parkinson’s: other movement disorders

The field of movement disorders extends to many other conditions.

Essential tremor: This is one of the most common movement disorders and causes involuntary shaking that worsens with activity. Unlike the tremor in Parkinson’s, it is often more noticeable when a person is using their hands to do something, like writing or eating.

Dystonia: This condition causes involuntary, sustained muscle contractions that lead to twisting and repetitive movements or abnormal postures.

Huntington’s disease: A genetic condition that causes involuntary movements, as well as psychiatric and cognitive problems.

Ataxia: This condition causes a lack of muscle control and coordination, leading to problems with balance and walking.

Expert care with Dr Dominic Paviour

Parkinson’s Disease is a progressive neurological disorder that affects movement, and in some cases, cognitive function. It occurs when nerve cells in the brain, which produce dopamine, become impaired or die. This loss of dopamine leads to the characteristic symptoms of Parkinson’s. Early detection and management of these symptoms are crucial for improving the quality of life for those affected. Let’s look more closely at the symptoms and finding a treatment plan for improved independence and wellbeing.

Tremor

Tremor, often the first noticeable symptom, typically begins in one hand, often described as a “pill-rolling” tremor. It may also affect the chin, lips, legs, or feet. The tremor is usually more prominent at rest and lessens during movement.

Bradykinesia

Bradykinesia, or slowness of movement, is another hallmark symptom. It makes everyday tasks, such as buttoning a shirt or walking, more difficult. Individuals may experience difficulty initiating movement or a general slowing down of physical actions.

Rigidity

Rigidity refers to stiffness or tightness of the muscles. It can occur in any part of the body and cause discomfort or pain. Rigidity often contributes to difficulty with movement and can lead to a decreased range of motion.

Postural Instability

Postural instability involves problems with balance and coordination. Individuals may experience unsteadiness, particularly when standing or walking. This can lead to an increased risk of falls.

Gait and Balance Problems

Changes in gait, such as shuffling or a stooped posture, are common. Individuals may also experience freezing, where they suddenly feel unable to move. Balance problems can further exacerbate difficulties with walking and increase the risk of falls.

Speech Changes

Parkinson’s can affect speech, leading to slurred, soft, or monotone voice. Individuals may also experience difficulty with articulation or hesitation before speaking.

Non-Motor Symptoms

In addition to motor symptoms, Parkinson’s can cause non-motor symptoms, including:

Sleep disturbances

Depression and anxiety

Constipation

Loss of smell

Cognitive changes

Parkinson’s Disease is a progressive neurological disorder that affects movement. A Consultant Neurologist plays a critical role in the diagnosis, management, and support of individuals living with this condition. Dr Dominic Paviour, MBBS PhD FRCP, a Consultant Neurologist Parkinson’s Disease patients may be referred to. He brings extensive experience and a patient-centred approach to his work with Parkinson’s patients.

The Role of a Neurologist in Parkinson’s Care

A neurologist specialising in Parkinson’s Disease is responsible for several key aspects of patient care. Firstly, they conduct thorough neurological examinations to diagnose the condition, often using imaging studies and clinical assessments. Secondly, they develop treatment plans that may include medication, physical therapy and lifestyle adjustments to manage symptoms. Thirdly, they monitor disease progression and adjust treatment strategies.

Neurologists also provide crucial support and education to patients and their families.

Parkinson’s Disease is a chronic and progressive neurodegenerative disorder that affects the nervous system. The condition is caused by the degeneration of dopamine-producing neurons in the brain, which leads to a range of motor symptoms, such as tremors, rigidity, and difficulty with movement. Parkinson’s Disease is one of the most common neurological disorders, affecting millions of people worldwide.

Diagnosis of Parkinson’s Disease

Diagnosing Parkinson’s Disease can be challenging. The symptoms can be similar to other conditions, such as essential tremor or multiple system atrophy. The diagnosis of Parkinson’s Disease is primarily based on a detailed medical history, physical examination, and a neurological examination. The doctor may also perform tests to rule out other conditions that could be causing the symptoms, such as blood tests, imaging studies, and other laboratory tests.

Parkinson’s Disease Treatment

Parkinson’s Disease treatment aims to improve the symptoms and quality of life of patients by managing the underlying causes and addressing the associated symptoms. There is currently no cure for Parkinson’s Disease, but several treatment options can help manage the symptoms and slow down the progression of the condition.

Medications

Medications are the primary treatment for Parkinson’s Disease. Dopamine replacement therapy is the most common treatment. It involves administering medications that mimic the effects of dopamine in the brain. Levodopa is the one of the most effective medications for Parkinson’s Disease, which is converted into dopamine in the brain. Other medications, such as dopamine agonists, MAO-B inhibitors, and anticholinergics, can also be used to manage the symptoms of Parkinson’s Disease.

Surgery

Surgery is an option for patients with advanced Parkinson’s Disease who do not respond well to medications. Deep brain stimulation (DBS) is a surgical procedure that involves implanting electrodes in the brain that stimulate specific areas to improve the symptoms of Parkinson’s Disease. Dr Paviour can refer patients to specialist neurosurgeons if they require surgical treatment

Physical Therapy

Physical therapy can help improve the motor symptoms of Parkinson’s Disease, such as balance, coordination, and mobility. Exercise can also help reduce the risk of falls, improve mood, and increase overall quality of life.

Speech Therapy

Parkinson’s Disease can affect speech and swallowing, making it difficult to communicate effectively. Speech therapy can help patients improve their speech and language skills, and also address swallowing difficulties.

Occupational Therapy

Occupational therapy can help patients with Parkinson’s Disease maintain their independence. It can also improve their ability to perform daily activities, such as dressing, bathing, and cooking. Occupational therapy can also help patients adapt to any changes in their physical abilities and improve their overall quality of life.

The right treatment for Parkinson’s Disease is critical for managing the symptoms and improving the quality of life of patients. Early diagnosis and appropriate treatment can slow down the progression of the condition and prevent further complications. While there is no cure for Parkinson’s Disease, several treatment options are available. Dr Paviour is a specialist who can help find the right plan for your needs, get in touch today to find out more.

A diagnosis of Parkinson’s Disease can be very worrying. It is vital you have the support of your healthcare team. Finding a neurologist to help with Parkinson’s Disease ensures you have the expert help you need. Neurologists specialise in nervous system and brain conditions, including Parkinson’s Disease.

Dr Dominic Paviour is a highly experience consultant neurologist. He supports and treats many patients with Parkinson’s Disease, at different stages of the condition.

Let’s discuss the condition in a little more depth.

Parkinson’s Disease

Parkinson’s Disease is the most common neurodegenerative cause of parkinsonism, representing approximately 80% of cases. It usually begins between the ages of 40 and 70 years, but it can happen sooner. Many people first realise they have the condition in the 70s. .

The cause of Parkinson’s Disease is not known but research shows it can be more common among relatives, and those with close family history of the condition at 2-3 times more likely to develop the disease themselves.

Parkinson’s Disease results in a loss of nerve cells in part of the brain. These lost cells lead to inhibition of dopamine production and dopamine is vital for regulating our bodies’ movement. This reduction in dopamine is responsible for many of the symptoms of Parkinson’s disease.

Common Symptoms of Parkinson’s Disease

People usually think of tremors and not much else when they think of Parkinson’s Disease. However, there is much more to the condition than this. Tremors, or involuntary shaking in different parts of the body, are often accompanied by slow movement and inflexibility and stiffness in the muscles.

Parkinson’s Disease can also effect your balance, memory and can lead to psychological symptoms such as anxiety, depression and insomnia. Each patient’s experience is different but any of these symptoms should lead to a visit to the GP and perhaps a neurology referral. Understanding the symptoms of Parkinson’s Disease is important so you recognise them.

Finding a neurologist to help with Parkinson’s Disease

Dr Dominic Paviour is a highly experienced consultant neurologist. He can help patients at any stage of Parkinson’s, as well as those who haven’t yet received a diagnosis. Initial appointments involve a full examination as well as discussion of your medical history. If you already have diagnosis you can also discuss your current treatment plan and Dr Paviour can talk through any alternative treatments or options available to you.

Simply get in touch to book your first consultation.

Parkinson’s Disease can be worrying, especially if you are concerned you have the condition or someone you love does. If you are worried about Parkinson’s, seek medical help as soon as possible. Not seeking the right medical help quickly can slow down the diagnosis process. Furthermore, you may find it is a different health issue you need treatment for. A diagnosis of Parkinson’s Disease is the first step in getting the right treatment as soon as possible and slowing the impact of the disease.

How do we diagnose Parkinson’s Disease?

Research from Parkinson’s UK confirms the disease is degenerative. It develops slowly, with symptoms becoming more pronounced over time. Symptoms often do not become obvious for many years. It is vital to look for symptoms described as Parkinsonism.

Parkinsonism describes several symptoms found in Parkinson’s Disease but may also appear in other conditions. Most patients with any form of Parkinsonism receive a diagnosis of idiopathic Parkinson’s disease. This is the most common form of the disease, and it’s called this because the cause is unknown. Other less common forms of Parkinsonism include vascular Parkinson’s and progressive supranuclear palsy.

Investigating Symptoms of Parkinson’s Disease

Parkinson’s will only be diagnosed after full consultation with a specialist neurologist. Your neurologist will take a full medical history and examine you, focusing on your nervous system. You should be ready to provide a history of your symptoms, particularly anything getting worse or changes. Some people begin to keep a diary of their symptoms so doctors can spot any patterns of degeneration.

The symptoms of Parkinson’s Disease differ for each patient. However, the most symptoms include the following:

Slowing movement

Difficulty turning in bed

Tremors

Fatigue

Smaller handwriting

Furthermore, there are additional early signs and symptoms of the disease, such as:

Unusual depression and anxiety

Disturbed sleep patterns

Loss of sense of smell

Constipation

Unusual tiredness

It is difficult for patients to recognise some of these early symptoms. They can indicate so many other conditions. However, if any of them occurs unexpectedly, you should visit your doctor.

Your consultant neurologist will examine you for the most common symptoms of Parkinson’s Disease, which may involve:

Testing your writing or drawing to check the size

Testing your walk to see if the swing of your arm differs and your stride length or speed

Speech testing to see if your voice lacks volume or is unusually soft

These are just some of the tests which help towards a diagnosis. Some patients trial medication and treatment to see if this helps in the diagnosis process.

Private Neurology Appointments for Parkinson’s Disease

Parkinson’s Disease usually begins between the ages of 40 and 70 years. Peak onset is in the seventh decade. Understanding Parkinson’s Disease Symptoms can assist in early diagnosis and management.

Parkinson’s Disease (PD) is the most common neurodegenerative cause of parkinsonism, representing approximately 80% of cases.

Several studies have indicated that PD is more common among relatives of index cases. When all studies are analysed, those with close family history of PD are 2 to 3 times more likely to develop the disease.

Parkinson’s Disease is caused by a loss of nerve cells in part of the brain. This leads to a reduction in a chemical called dopamine in the brain. Dopamine plays a vital role in regulating the movement of the body. A reduction in dopamine is responsible for many of the symptoms of Parkinson’s disease.

However, there can be various factors contributing to PD. These may include contributions genetics, environmental toxins as well and aging.

Parkinson’s Disease Symptoms

The pathological changes of PD may appear as early as three decades before the appearance of clinical signs.
The NHS specifies symptoms that are associated with PD. The main indicators of Parkinson’s Disease Symptoms are involuntary shaking of particular parts of the body which is also known as a tremor. In addition, a patient may experience slow movement as well as stiff and inflexible muscles.

A person with Parkinson’s Disease Symptoms can also display a wide range of other physical and psychological signs. These can include depression and anxiety and memory problems. Balance, insomnia and loss of sense of smell may also be evident.

Symptoms can present at different times and are different for each patient so it always recommended to seek medical advice should you present with any of these signs.

There is currently no cure for Parkinson’s Disease. However, treatments are available to help reduce the main symptoms so it is important to speak to your GP who may refer you.
