Multiple sclerosis (MS) is a chronic autoimmune disease that affects the central nervous system, disrupting the flow of information between the brain and the body. Dr Dominic Paviour provides the multiple sclerosis treatment London patients are looking for, through a range of options. Living with MS can present significant challenges, but advancements in treatment have brought new hope and improved quality of life for many individuals.

MS is different for each person, with a wide range of symptoms, including fatigue, numbness, tingling, muscle weakness, vision problems and cognitive difficulties. The unpredictable nature of MS can make it difficult to manage. This further highlights the importance of seeking expert medical care.

Understanding Multiple Sclerosis and its Treatment

MS occurs when the immune system attacks the myelin sheath, the protective covering of nerve fibres. This damage disrupts nerve signals which leads to the various symptoms associated with MS. While there is currently no cure for MS, a range of treatments are available to manage symptoms, slow disease progression and improve wellbeing.

The treatment approach for MS is tailored to the individual’s specific needs and disease course. It often involves a combination of:

Disease-Modifying Therapies (DMTs): These medications aim to reduce the frequency and severity of relapses, slow the accumulation of disability, and prevent further damage to the central nervous system.

Symptom Management: Addressing individual symptoms is essential for improving quality of life. This may involve medication for pain, fatigue, muscle spasms, bladder dysfunction, or other specific concerns. Physiotherapy, occupational therapy and speech therapy can also play a role in managing symptoms.

Rehabilitation: Rehabilitation programmes can help individuals with MS regain lost function, adapt to physical limitations, and improve their independence.

Lifestyle Modifications: Making healthy lifestyle choices can positively impact the course of MS. This includes maintaining a balanced diet, engaging in regular exercise, managing stress, and avoiding smoking.

These specialists play a crucial role in helping individuals and their families understand and cope with the challenges posed by dementia. Dr Dominic Paviour is a consultant neurologist for Dementia and many other neurological conditions.

Diagnosis and Evaluation

One of the primary responsibilities of a consultant neurologist for dementia care is to provide accurate and timely diagnoses. This involves conducting a comprehensive assessment of the patient’s medical history, mental status, and neurological function. The neurologist may also order additional investigations, such as brain scans or cognitive tests, to gather further evidence and rule out other potential causes of the patient’s symptoms.

Developing a Treatment Plan

Following a diagnosis, your consultant neurologist works closely with you and your family to develop an individualised treatment plan. While there is no cure for dementia, various medications and non-pharmacological interventions can help manage symptoms. Furthermore they can slow disease progression, and improve overall quality of life.

Medication Management

Consultant neurologists are well-versed in the intricacies of dementia medications, their potential side effects, and interactions with other medications. They can also prescribe appropriate medications, monitor their effectiveness, and adjust dosages as needed to optimise symptom control.

Non-Pharmacological Interventions

Beyond medications, consultant neurologists may recommend non-pharmacological interventions to complement the patient’s treatment plan. These may include cognitive stimulation therapy, lifestyle modifications, and behavioural interventions to maintain cognitive function and support daily living skills.

Monitoring Disease Progression

Regular consultations with a consultant neurologist are essential for monitoring the progression of dementia and adjusting the treatment plan as needed. The neurologist will assess the patient’s symptoms, cognitive function, and overall wellbeing. This helps to ensure that the treatment remains effective and tailored to the patient’s needs.

Collaborative Care

Consultant neurologists often work collaboratively with other healthcare professionals. They work together on the patient’s care and this may include collaborating with geriatricians, psychologists, and social workers. Furthermore, this team-based approach ensures that the patient receives comprehensive and coordinated care that addresses their physical, psychological, and social needs.

Your GP is responsible for managing your general health, but they are not a specialist in any specific area of medicine. If you visit your GP with a number of symptoms, you may need referral to a specialist. To answer the question, why would I be referred to a neurologist, it’s worth looking at the symptoms that may lead to this.

Many conditions and symptoms require specialist diagnosis and treatment. This is where specialist medical departments come into play, including neurology. Neurology covers many different conditions relating to the nervous system including migraine, seizures, Parkinson’s disease, multiple sclerosis, and stroke. Neurologists can assess your neurological symptoms, reach a diagnosis and help design an effective treatment plan.

Referral to Neurology

A neurologist is specially trained in treating conditions affecting the nervous system. The nervous system includes your brain, nerves and spinal cord as well as related muscles and blood circulation. Conditions that impact the nervous system and related areas can cause chronic pain, memory loss, speech problems and more. These conditions often require testing and treatments that a GP is not qualified or experienced in.

Symptoms that lead to neurology referral include:

Persistent or chronic headache

Unexplained numbness

Chronic pain with no obvious cause

Tremors or uncontrolled shaking

Memory loss

There are many other symptoms that may need neurological attention that your GP can advise and arrange necessary referrals.

Neurological Conditions

Neurologists treat a far-reaching range of conditions including head injuries, chronic headaches and migraine and specific disorders and syndromes.

Multiple sclerosis is also another neurological condition that results in chronic pain and many additional symptoms. MS is also a progressive and degenerative disease that impacts the brain and spinal cord, but the condition can be managed with the right treatment plan from your neurologist.

Similarly, seizures and epilepsy should involve regular neurology visits. If you experience a seizure then it is likely you will need a neurology referral. Neurologists can also diagnose epilepsy and carry out other testing necessary to find the cause of any individual seizure.

Other neurological symptoms such as memory loss and dizziness could be caused by several conditions. An experienced neurologist will work with you to find the cause of your symptoms, provide a diagnosis where necessary and find a way to manage and alleviate your symptoms.

London benefits from being one of the most populous cities in the world. It makes it easy to find almost any service you could need. Therefore, if you’re looking for a consultant neurologist in London there will be many choices. This makes it even more important to spend time selecting the right professional for your requirements.

Neurology and Neurological Symptoms

Neurology is a specific field of medicine that looks at nervous system disorders and conditions. The nervous system is made up of the central nervous system and the peripheral nervous system. A consultant neurologist can help diagnose and treat symptoms of both.

Neurologists specialise in treating conditions in these parts of the body. Disorders which affect the brain, spinal cord and nerves include:

Specific spinal cord disorders

Some speech and language disorders

Parkinson’s Disease and related movement disorders

Cerebrovascular diseases including strokes

Headache conditions such as migraine

Infections of the brain and nervous system

Alzheimer’s Disease and similar neurodegenerative conditions

Epilepsy and other seizure disorders

Multiple sclerosis and other demyelinating diseases

Visiting a Consultant Neurologist in London

Most people only visit a neurologist after referral from their GP. You may also need a referral to a consultant neurologist if you are experiencing:

Headaches: patients who suffer from painful headaches including migraine may need to see a neurologist for further tests.

Chronic neck and back pain: if this is alongside other neurological symptoms it is important to explore in more depth.

Dizziness: when dizziness is experienced as vertigo or disequilibrium, you may need to see a neurologist

Numbness: numbness or tingling can be caused by many medical problems. However, if it comes on quickly or affects one side of the body then it is worth seeking a neurology referral

Your GP or general healthcare team may also refer you to neurology for other reasons. Understandably, it can be daunting but an experienced consultant neurologist like Dr Paviour will be able to help and find the right treatment plan.

If you have seen a GP or a Consultant who has advised you should have an MRI Scan, you may wonder what this involves so read on to find out more.

How is an MRI Scan Different from an X-Ray?

An X-Ray uses radiation to take a picture of the inside of a body. This is often more readily available than an MRI Scan and can quickly diagnose an injury or mass. An MRI Scan offers a far more detailed view of tissue and organs so is ideal for looking at the brain.

What is an MRI Scan?

MRI stands for Magnetic Resonance Imaging. This is a type of scan that uses strong magnetic fields and radio waves to take pictures of the spine or brain. The technology helps distinguish between different tissues so the result is a detailed picture that a Consultant can use to diagnose a range of conditions.

Who Can have an MRI Scan?

The NHS states that Magnetic resonance imaging (MRI) is very safe and most people are able to have the procedure. However, in some instances, an MRI scan may not be recommended. You can find out more information here.

What Happens During an MRI Scan?

The scan does not hurt at all as you will lie still on a bed that moves into the scanner. The Radiographer will explain the process but will move to a different room to operate the scanner. They will talk to you through the inbuilt intercom and at times it will make a loud tapping noise as the electric current is turned on and off.
The whole process takes less than an hour and a half. Afterwards, your Consultant will be provided with the images so they can make an accurate diagnosis.

What Happens After the MRI Scan?

The scan is usually carried out during a day clinic so you won’t need to stay in hospital unless you are already an inpatient. You can carry on as normal afterwards unless advised otherwise or you have had a sedative.
Your Radiologist or Consultant will review the scan and discuss with colleagues to agree on the diagnosis and treatment plan. Your GP or Consultant will then discuss the results with you, usually 2-3 weeks later.

Dr Paviour’s Approach

Dr Paviour uses the latest technology to diagnose and find treatment for neurological conditions. He will discuss your symptoms and medical history during your private consultation as well as carry out a physical examination and relevant tests. These might include blood tests, imaging of the nervous system (MRI Scan or CT Scan) and electrical testing of nerves and muscles (EMG) or brain function (EEG).

Dr Paviour is highly rated as a neurologist in London, not only by his peers but most importantly, by his patients. Feel free to contact us so we can discuss any symptoms and how Dr Paviour can help.

What is a Neurologist?

A neurologist is a medically qualified doctor dealing with disorders of the nervous system. This includes the diagnosis and treatment of all categories of disease that involve the central, peripheral and autonomic nervous systems.

Neurologists also manage conditions that affect the coverings of the brain (the meninges), the blood supply of the nervous system and muscles (as these are controlled by the nervous system).

Dr Paviour has many years experience in supporting patients with various medical conditions. These include migraines, multiple sclerosis and epilepsy. Dr Paviour will undertake an initial consultation to understand the nature of the main symptoms you experience. He will review any previous medical problems you may have had and enquire about current and past medical treatments.

Dr Paviour may enquire about any conditions that run in the family. You will have a physical examination that will focus on the functioning of the nervous system so he has a full picture to be able to diagnose.

Electrical activity is happening in our brain all the time. A seizure happens when there is a sudden burst of intense electrical activity in the brain. This patient guide aims to give some information on epilepsy and seizures to help understand the condition.

What are Epilepsy and Seizures?

There are many different types of seizure. They can be classed by where in the brain the epileptic activity starts.

Focal (partial) seizures – In these seizures, the epileptic activity starts in just a part of your brain. You may stay alert in this type of seizure. Sometimes, epileptic activity starts as a focal seizure, spreads to the rest of your brain and becomes a generalised seizure.

Generalised seizures – These seizures involve epileptic activity in both halves of your brain. You lose consciousness during this type of seizure, although sometimes it can be so brief that no one notices. The muscles in your body may stiffen and/or jerk. You may fall down.

What Triggers a Seizure?

Some things make seizures more likely for some people with epilepsy. These are often referred to as ‘triggers’. These can be things like stress, not sleeping well and drinking too much alcohol. Some people say they have more seizures if they miss meals. Not taking your epilepsy medicine is another common trigger. A very small number of people with epilepsy have seizures triggered by lights that flash or flicker.

Diagnosis and Treatment

One of the most important parts of getting the right treatment for your seizures is getting the right diagnosis. Doctors base their diagnosis on what they are told about your seizures so keeping a record is important. With this information, they can refer you to a neurologist. The neurology team can then assess your condition so they can provide a treatment plan, often with the use of medication.
